Wegovy and Ozempic at Costco: the price without insurance, dose by dose

Evidence-based

Written by the VitalDecades editorial team. Last updated . How we source.

Costco's member price for Wegovy is $199 for two fills then $349 a month for the pen, and $149 to $299 for the tablets; Ozempic is $199 then $349, or $499 at 2 mg. Every price matches Novo Nordisk's own self-pay price.

Without insurance, Costco charges members $199 for each of the first two months of the Wegovy pen and $349 a month after that ($399 for Wegovy HD), and $149 to $299 a month for Wegovy tablets depending on dose. Ozempic is $199 for two months and then $349 a month for the pen, $499 at the 2 mg dose, and $149 to $299 for the tablets. We read every price on Costco's own pharmacy site on October 28, 2026, and each one matches Novo Nordisk's own self-pay price for the same dose, so Costco is a convenient place to buy at the manufacturer's price rather than a discount on it.

Costco's prices, dose by dose

We read these on Costco Pharmacy's own price search on October 28, 2026, for every strength of Wegovy, Wegovy HD and Ozempic it lists, and checked them against Novo Nordisk's own self-pay terms read the same day. Costco shows the same price for warehouse pick-up and for home delivery. A month is one box of four pens for Wegovy, one pen for Ozempic, and a bottle of 30 tablets for either pill.

Costco Member Value Pricing from rx.costco.com price search and Member Value Pricing page; Novo Nordisk self-pay prices from the Wegovy and Ozempic savings-offer terms on NovoCare. All read October 28, 2026. Costco prices are for members paying cash outside insurance.

Wegovy pen, 0.25 or 0.5 mg (4 pens)$199, limited to 2 fills, then $349$199 for the first 2 fills (new patients, through December 31, 2026), then $349
Wegovy pen, 1, 1.7 or 2.4 mg (4 pens)$349$349
Wegovy HD pen, 7.2 mg (4 pens)$399$399
Wegovy tablets, 1.5 mg (30)$149$149
Wegovy tablets, 4 mg (30)$199$199
Wegovy tablets, 9 or 25 mg (30)$299$299
Ozempic pen, 0.25 or 0.5 mg doses (1 pen)$199, limited to 2 fills, then $349$199 for the first 2 fills (new patients, through December 31, 2026), then $349

Ozempic tablets, 4 mg (30)$199$199
Ozempic tablets, 9 mg (30)$299$299

The $199 starting price is a two-fill introduction, not a monthly price. Costco's own price cards for both pens say "Limited to 2 fills" and give the price as "$349/mo after" December 31, 2026. Novo's terms set the same limit and define who gets it: a new patient is one who has not redeemed a Wegovy (or Ozempic) savings offer in the past 365 days, and the prescription must be filled by December 31, 2026. Costco's price search, on its own, shows $199 for the starter pens without mentioning the two-fill limit, so expect $349 from the third box.

Who gets the Costco price

Members. Costco calls this Member Value Pricing, and its FAQ says "all Costco Members are eligible" with nothing to sign up for. On non-members it is direct: they are welcome at the pharmacy, "while Non-Members pay the cash price." Costco does not publish that cash price on its site, so a non-member has to call the pharmacy for it.

Paying cash, outside insurance. Costco's terms say the program "cannot be combined with insurance", and its FAQ rules out any prescription billed to a government program or to commercial insurance, along with discount cards, coupons and promotions. If your plan covers the drug, compare its copay with the member price and use whichever is lower; Costco says it can be used when a medication is not covered by your insurance.

Delivery. Costco's pricing footer says shipping and handling are "not included in the price", but its mail-order pharmacy page says "Costco offers free standard shipping. Expedited shipping options are available for an additional fee." Novo's own home-delivery pharmacy advertises free delivery at the same prices. At standard shipping or warehouse pick-up the cost is the same either way; only Costco's expedited shipping adds to it.

Two things on Costco's page that are wrong or unresolved

The "Ozempic Tablets" cards at 1 mg and 2 mg are pen prices. Costco's Member Value Pricing page shows two cards titled Ozempic Tablets: $349 "per month (1mg)" and $499 "per month (2mg)". Ozempic tablets are not made in those strengths: Novo's terms list them at 1.5, 4 and 9 mg, and Costco's own price search prices those at $149, $199 and $299. The $349 and $499 figures are what Costco's search charges for the Ozempic 1 mg and 2 mg pens. If you take the Ozempic pill, your price is the tablet row in the table above, not those cards.

The Wegovy 1.5 mg tablet may or may not rise in January. Costco's card prices it at $149 with the note "Price is $199/mo after 12/31/26." Novo's own terms, read the same day, list the 1.5 mg bottle at $149 as a standard price with no end date. We cannot tell you which is right for January 2027; check the price at your first refill of the new year.

How Costco's price relates to Novo Nordisk's own

The numbers line up with Novo Nordisk's own self-pay program, and Novo names Costco as a participant in it. Its November 17, 2025 announcement, which cut the standard self-pay price of both pens "from $499 to $349 per month" (Ozempic 2 mg stayed at $499), named "Costco, GoodRx, WeightWatchers, Ro, LifeMD, and eMed" among the "participating organizations" offering the self-pay prices. Any Costco price you saw before that date, or in coverage written before it, is out of date.

Novo's own offer carries a condition that Costco's pages do not mention: the patient must have been "prescribed Wegovy for an FDA-approved indication within Wegovy's labeling", and the same for Ozempic. Neither company says whether Costco's member price runs through Novo's offer or is set by Costco's own program, which Costco says is administered by Costco Pharmacy and Ventegra. Ask the pharmacist which applies before your first fill, especially if you are a non-member or your prescription is for Ozempic.

What a year costs

By our arithmetic from the prices above, a new Wegovy pen patient who reaches 2.4 mg pays $199 twice and then $349, so the first 12 boxes cost $3,888 ($398 plus ten boxes at $349). Each box is a 28-day supply, so a full calendar year takes 13. Someone on Wegovy tablets at 9 or 25 mg pays $299 a bottle, $3,588 for 12. An Ozempic patient on the 2 mg pen pays $499 a pen, $5,988 for 12. None of this includes a prescriber's fee if you use an online program.

What to do next

Check insurance first. If a commercial plan covers Wegovy, Novo's savings offer can bring the copay to as little as $25 a month; because its savings are capped at $100 a month, it beats the $349 cash price whenever your plan's copay is under $449 (our arithmetic). Our prior authorization guide sets out what insurers require to approve it.

If you are paying cash, the price is the same at Costco and at Novo's own pharmacy, so choose on convenience: Costco for warehouse pick-up or its mail order if you are a member, NovoCare's home delivery if you are not. Our NovoCare Pharmacy page covers how that route works and which programs prescribe into it, and the GLP-1 cost calculator totals the months you choose. For the full dose grids and list prices, see our Wegovy and Zepbound cost guide and Ozempic cost guide, and for the pill against the pen, Wegovy pill vs injection.

Frequently asked questions

How much is Wegovy at Costco?

For members paying cash, read October 28, 2026: $199 for each of the first two boxes at 0.25 or 0.5 mg, then $349 a month at every pen dose through 2.4 mg, and $399 for Wegovy HD 7.2 mg. Tablets are $149 at 1.5 mg, $199 at 4 mg and $299 at 9 or 25 mg.

How much is Ozempic at Costco?

The pen is $199 for each of the first two fills at the 0.25 or 0.5 mg doses, then $349, and $499 at 2 mg. Ozempic tablets are $149, $199 or $299 for 1.5, 4 or 9 mg. Those are member prices for cash payers, read October 28, 2026.

Is Costco cheaper than NovoCare?

No. Every strength we checked was the same price at both. Costco's mail-order pharmacy offers free standard shipping (expedited costs extra) and NovoCare advertises free delivery, so at standard shipping or warehouse pick-up the cost is the same.

Do I need a Costco membership?

For these prices, yes. Costco says non-members can use the pharmacy but pay the cash price, which it does not publish online. Call your local Costco pharmacy for it.

Can I use my insurance and the Costco price together?

No. Costco says Member Value Pricing cannot be combined with insurance, including commercial plans and government programs. You choose one or the other for each prescription.
