The two prices, and which one is yours
List price (wholesale acquisition cost, WAC) is what the manufacturer charges wholesalers; it drives insurance negotiations and the sticker at a pharmacy counter without coverage. Self-pay is the direct-to-patient price through the manufacturer's own pharmacy program (NovoCare for Novo Nordisk, LillyDirect for Lilly), also available through participating pharmacies and telehealth programs. If you have no coverage for the drug, self-pay is your price. If you have commercial coverage, the savings card is. Government coverage (Medicare, Medicaid, VA, TRICARE) excludes you from savings cards; see the coverage guide for what applies instead.

List price for the Wegovy pill, pen and HD pen is $1,349.02 per package. On February 24, 2026 Novo Nordisk announced that on January 1, 2027 the list price of Wegovy 2.4 mg injection and 25 mg tablets falls to $675, a change the company said does not affect self-pay prices.

Lilly's list-price pages give Zepbound as $499 to $1,086.37 per fill and Mounjaro as $1,112.16 (WAC as of January 1, 2026), and Foundayo as $649 for a 30-day supply. Zepbound's regular direct-to-patient prices without the refill program are $299, $399, $499, $699, $699 and $699 by dose.
If you have commercial insurance that covers the drug
Novo's Wegovy Savings Offer: pay as little as $25 for a one-month fill of the pen or tablets, with a maximum saving of $100 per month ($200 for two months, $300 for three). Lilly's Zepbound Savings Card: as little as $25 for up to a three-month fill of the single-dose pen, maximum $100 per month and $1,300 per calendar year, up to 13 fills, expiring December 31, 2026. If your commercial plan does not cover Zepbound, the card brings the single-dose pen to $499 and the KwikPen to $299 to $449 by dose. Both programs exclude anyone on Medicare, Medicaid, VA, DoD or TRICARE coverage.
TrumpRx.gov
TrumpRx.gov, launched February 5, 2026, lists Wegovy pill $149, Wegovy pen $199, Ozempic $199 and Zepbound $299 as its headline monthly prices, with the same offer text as the manufacturers (Wegovy pen $199 for two fills then $349; Ozempic $199 for two fills then $349, or $499 for 2 mg) and routes Zepbound, Foundayo and Mounjaro orders to LillyDirect. It is a directory to the manufacturer programs above, not a lower price.
Patient-assistance programs
Neither manufacturer's charity program covers its weight-loss GLP-1 today. Novo Nordisk's Patient Assistance Program product list for 2026 includes Ozempic (for uninsured patients only) but not Wegovy, Saxenda, Rybelsus or Victoza; Lilly Cares lists Trulicity as its only GLP-1 and does not include Zepbound, Mounjaro or Foundayo.
The arithmetic that matters
Doses escalate, so a year is not twelve times the starting price. A Zepbound patient who reaches a maintenance dose pays $449 a month on the refill program, about $5,400 a year, or up to $8,400 if the 45-day condition lapses; a Wegovy pen patient pays $349 after the two-month offer, about $4,100 in the first year; Wegovy tablets at 25 mg run $299, about $3,600. Compounded programs advertise less, and sell a product the FDA has not reviewed; that trade-off is set out in our explainer.
