Does insurance cover GLP-1s? Medicare, Medicaid, employers

As of August 2026: Medicare Part D still cannot cover drugs used solely for weight loss by statute, but the Medicare GLP-1 Bridge (July 1, 2026 to December 31, 2027) gives eligible beneficiaries Wegovy, Wegovy tablets, Zepbound KwikPen or Foundayo for a $50 copay; Wegovy is Part D-coverable for its cardiovascular indication. Thirteen state Medicaid programs covered GLP-1s for obesity in January 2026 (KFF). Around 43 to 49% of large employers cover them for weight loss.

Coverage of GLP-1s for weight loss changed more in the year to August 2026 than in the previous decade: Medicare now has a $50-a-month bridge program, Medicaid coverage shrank to 13 states, and large-employer coverage rose to about half. Here is where each payer stands, from the agency and survey documents.

Medicare

The rule that has governed for two decades: the Part D definition of a covered drug excludes agents that Medicaid may exclude, which includes drugs used for weight loss. CMS proposed reinterpreting that in late 2024 and, in its April 2025 final rule for 2026, did not finalise it. Two doors are open anyway. First, a GLP-1 with a medically accepted indication other than weight is coverable: Wegovy for cardiovascular risk reduction (since March 2024), and Ozempic, Mounjaro, Rybelsus and Zepbound for their diabetes, sleep apnoea or other indications, subject to your plan's formulary and prior authorization.

Second, the Medicare GLP-1 Bridge. Announced with the November 2025 manufacturer agreements and CMS's BALANCE model, and launched July 1, 2026, it is a demonstration under section 402 that runs to December 31, 2027 (extended from its original end date in an April 2026 CMS memo). Eligible drugs, when used to reduce excess body weight: Foundayo, Wegovy injection and tablets, and Zepbound KwikPen only (not single-dose vials or pens). The copay is $50 per one-month supply, paid outside the Part D benefit, so it does not count toward the deductible or out-of-pocket maximum and Extra Help does not reduce it. Clinical criteria: age 18 or older and BMI 35 or higher; or BMI 30 or higher with heart failure with preserved ejection fraction, uncontrolled hypertension or chronic kidney disease stage 3a or above; or BMI 27 or higher with prediabetes, a previous heart attack or stroke, or symptomatic peripheral artery disease. Beneficiaries whose diagnosis (type 2 diabetes, moderate to severe OSA, MASH) already qualifies for Part D coverage use Part D instead. Prior authorization, once approved, is valid through December 31, 2027 unless you change drugs.

The Part D portion of the BALANCE model was delayed for 2027 pending evaluation; the Bridge is the Medicare route for weight-loss use through 2027.

Medicaid

State Medicaid programs may cover anti-obesity drugs but are not required to, and coverage moves with state budgets. KFF, a tracker rather than a primary source, counted 13 state fee-for-service programs covering GLP-1s for obesity as of January 2026, down from 16 in October 2025 after California, New Hampshire, Pennsylvania and South Carolina dropped coverage and North Carolina reinstated it. The November 2025 agreements give state Medicaid programs access to the manufacturers' Medicare prices, and CMS's BALANCE model lets states join between May 2026 and January 2027; CMS's model page listed no participants as of August 14, 2026. For your state, the primary source is the state Medicaid preferred drug list, not any tracker.

Employer and commercial plans

Two 2025 surveys, both self-described as surveys rather than a census: KFF's Employer Health Benefits Survey found 16% of firms with 200 to 999 workers, 30% with 1,000 to 4,999, and 43% with 5,000 or more covered GLP-1s for weight loss in 2025 (up from 28% for the largest firms in 2024), with 34% of covering firms requiring a dietitian, case manager or lifestyle program; Mercer's survey of 2,010 employers put large-employer coverage at 49%, up from 44%. Where a plan covers the drug, prior authorization is standard and usually mirrors the label criteria; the manufacturers' savings cards then bring the copay to as little as $25 with monthly caps (see the cost guide).

What to do with this

  • Medicare, weight-loss use: ask your prescriber about the Bridge criteria and the drug list (Zepbound is KwikPen only). If you have type 2 diabetes, OSA or MASH, the drug goes through Part D instead.
  • Medicare, cardiovascular use: Wegovy is Part D-coverable for established cardiovascular disease with obesity or overweight; check your plan's formulary.
  • Medicaid: check your state's preferred drug list, and ask whether the state has joined BALANCE.
  • Employer plan: ask HR or the plan whether GLP-1s for weight loss are covered, whether a lifestyle program is required, and what the prior-authorization criteria are; then apply the savings card.
  • No coverage: the manufacturer self-pay programs are the price.

When to talk with a clinician

Prior authorization runs on documentation: BMI, comorbidities, prior weight-management attempts. A clinician who knows your record can file it once and correctly. Programs that offer an insurance concierge do this for a fee; our ranked programs say which ones do.

Frequently asked questions

Does Medicare cover Wegovy or Zepbound for weight loss?

Not through the Part D benefit, which excludes weight-loss drugs by statute. From July 1, 2026 to December 31, 2027 the Medicare GLP-1 Bridge gives eligible beneficiaries Wegovy (pen or tablets), Zepbound KwikPen or Foundayo for a $50 copay outside Part D. Wegovy is Part D-coverable for cardiovascular risk reduction.

Does Medicaid cover Ozempic or Wegovy for weight loss?

It depends on the state. KFF counted 13 state programs covering GLP-1s for obesity in January 2026. Your state's preferred drug list is the source to check.

Will my employer plan cover a GLP-1 for weight loss?

Roughly half of the largest employers did in 2025 (43% in KFF's survey for firms of 5,000+; 49% in Mercer's), and many require a lifestyle program. Ask the plan directly.

Can I use a savings card with Medicare?

No. The Wegovy and Zepbound savings programs exclude anyone with federal or state health coverage. The Bridge's $50 copay is the Medicare route.
