High ALT on a blood test: what the number means and what to do next

The American College of Gastroenterology puts the true healthy upper limit for ALT at 29 to 33 IU/L for men and 19 to 25 IU/L for women, below what many laboratories flag. The commonest cause of a mildly high result is fat in the liver, and the question that actually matters is not the enzyme, it is whether the liver has scarred.

ALT is the liver test most likely to come back flagged, and the least likely to be explained. It is an enzyme that lives inside liver cells, so a raised level in blood means liver cells have been leaking it. What it does not tell you, on its own, is why, how long it has been happening, or whether any lasting harm has been done. Those three questions are the whole of the workup, and you can get a long way through them with information you already have.

What ALT actually measures

Alanine aminotransferase is an enzyme that sits inside hepatocytes, the working cells of the liver. When those cells are injured or stressed, some of the enzyme escapes into the bloodstream and the blood level rises. The ACG guideline is careful about the vocabulary here: these are liver chemistries or liver tests, not "liver function tests", because ALT measures leakage rather than function. A liver can be doing its job well with a raised ALT, and a badly scarred liver can produce a perfectly ordinary one.

ALT is the more liver-specific of the two aminotransferases. AST is also found in heart, skeletal muscle, kidney and red blood cells, which is why an isolated AST rise with a normal ALT so often turns out to be muscle rather than liver. The ACG frames the pattern in terms of which enzyme group is disproportionately raised: ALT and AST up out of proportion to alkaline phosphatase points to hepatocellular injury, while alkaline phosphatase up out of proportion to the aminotransferases points to a cholestatic problem in the bile ducts.

The threshold your lab prints is probably not the healthy one

This is the single most useful thing to know about the test. Reference intervals are built from the spread of results in a reference population, and in most countries a good share of that population has undiagnosed fatty liver. The upper limit inherited from that approach is therefore higher than the level a genuinely healthy liver produces. The ACG guideline addressed this directly and published a healthy normal derived from people screened to exclude liver disease.

Two practical consequences follow. First, a result your printout calls normal can still sit above the healthy range, particularly for women, whose healthy ceiling is roughly ten points lower than men's. Second, a result flagged as high by a lab using a generous ceiling has already cleared a bar the ACG would treat as worth looking at. What actually causes it

The ACG evaluation of hepatocellular injury is a fixed list, and it is worth knowing what is on it so you can tell whether your own workup was complete. It includes testing for viral hepatitis A, B and C, assessment for fatty liver disease and alcohol-associated liver disease, and screening for hereditary haemochromatosis, autoimmune hepatitis, Wilson's disease and alpha-1 antitrypsin deficiency. The guideline adds that a history of prescribed and over-the-counter medicines should be sought, which in practice is the step most often skipped.

In an adult with a mild, persistent elevation and no other symptoms, the probability heavily favors metabolic dysfunction-associated steatotic liver disease, or MASLD, the condition renamed from non-alcoholic fatty liver disease in 2023. The American Gastroenterological Association's clinical care pathway put its prevalence at approximately 37 percent of US adults. It is largely asymptomatic, which is why it is usually found through exactly this route: an enzyme flagged on a routine panel.

Several causes are easy to miss and worth ruling out before anyone books an ultrasound. Strenuous or unaccustomed exercise raises both aminotransferases from muscle, and a creatine kinase measured at the same time separates muscle from liver in a single step. Alcohol intake is routinely under-reported, and the care pathway notes that even low intake raises the risk of advanced liver disease in people who already have fatty liver. Medicines and supplements are a large category: paracetamol, some antibiotics, statins, anti-epileptics and a long list of herbal and bodybuilding supplements can all do it.

Why the size of the number is not the interesting part

People understandably fixate on how far above the line their result sits. The clinically important question is different: is there scarring, and is it advanced? Fibrosis is what determines outcomes in fatty liver disease, and it is not readable from the enzyme.

The standard first-line answer is FIB-4, a score calculated from four things you very likely already have: age, ALT, AST and platelet count. It requires no new test and no imaging. The AGA clinical care pathway relies on it because it has the best diagnostic accuracy for advanced fibrosis among the simple non-proprietary scores.

The over-65 adjustment exists because FIB-4 rises with age on its own, so the standard cutoff sends too many older people to unnecessary follow-up. If your score has been calculated without that adjustment and you are over 65, it is a fair thing to raise.

Does a normal ALT mean you can stop worrying?

Not automatically, but the evidence here is more reassuring than the usual internet answer. A cohort study in Hepatology followed people with hepatic steatosis and separated them by whether their ALT was persistently normal or elevated. Those with steatosis and a normal ALT had a cirrhosis incidence of 1.22 per 1,000 person-years, against 3.85 in the group with steatosis and a raised ALT. After adjustment, steatosis with an elevated ALT carried a 3.37-fold higher risk of developing cirrhosis than steatosis with a normal one, and the risk in the normal-ALT group was not significantly different from people with no steatosis at all.

Read carefully, that study cuts both ways. It is a reason not to panic about fatty liver found incidentally alongside a normal enzyme, and it is a reason to take a persistently raised ALT seriously rather than filing it as a lab quirk. Two caveats belong with it: the cohort was over 90 percent men, drawn from a US veterans population, and the finding is about averages rather than any individual liver.

What to do with a high result

A sensible order of operations, all of it low cost.

  • Repeat it before acting. A single elevation can follow a virus, a heavy training week, a course of medication or a weekend of drinking. The ACG treats persistence, not a single reading, as the trigger for evaluation.
  • Write down everything you take. Prescriptions, over-the-counter painkillers, protein powders, herbal preparations, pre-workouts and anything bought online. This list explains a meaningful share of unexplained elevations and costs nothing to produce.
  • Get the score, not just the enzyme. Ask whether FIB-4 was calculated. The inputs are on the same panel, and it converts an ambiguous number into a risk category.
  • Be honest about alcohol. The threshold at which alcohol contributes is lower than most people assume once fatty liver is present.
  • Ask whether the standard causes were excluded. Hepatitis B and C serology, iron studies, and, where the picture fits, autoimmune and metabolic causes.

If the underlying problem is metabolic, the treatment is metabolic. Weight loss, a Mediterranean-style eating pattern, and treating the diabetes, blood pressure and lipids that travel with it are what the care pathway recommends for the low-risk majority. Frequently asked questions

What is a dangerously high ALT?

There is no single number that separates safe from dangerous, because context does most of the work. Very large elevations, in the hundreds or thousands, point towards acute injury such as a drug overdose, acute viral hepatitis or an interruption of blood flow to the liver, and are assessed urgently. A mild persistent elevation is a different clinical problem, evaluated over weeks rather than hours. Raised ALT with jaundice, confusion, severe abdominal pain or easy bruising should be seen the same day.

Can exercise raise ALT?

Yes. Strenuous or unaccustomed exercise releases aminotransferases from skeletal muscle, and this is a frequent cause of a puzzling result in people who train. Measuring creatine kinase at the same time usually settles it, and repeating the test after several days without heavy training is the simplest confirmation.

Does a normal ALT rule out liver disease?

No. Liver disease, including advanced fibrosis, can exist with a normal enzyme, which is why fibrosis scores exist. That said, in people with fatty liver, a persistently normal ALT was associated with a substantially lower rate of progression to cirrhosis than an elevated one.

How quickly can ALT come down?

It depends entirely on the cause. Withdrawal of a causative drug or a period without alcohol can normalize it within weeks. Improvement driven by weight loss in fatty liver is measured in months. What matters more than the speed is whether the level is trending down on repeat testing while the underlying cause is addressed.

Is ALT the same as SGPT?

Yes. SGPT, or serum glutamic pyruvic transaminase, is an older name for the same enzyme, and some laboratories still use it. If your report says SGPT, read it as ALT.

When to talk with a clinician

Book a review for any ALT that stays above the healthy range on a repeat test, and sooner if it is several times the upper limit, if you have symptoms such as jaundice, dark urine, pale stools, right upper abdominal pain or unexplained weight loss, or if you take a medicine known to affect the liver. Bring the list of everything you take, an honest account of alcohol, and the previous results so the trend is visible. Ask two specific questions: what was my FIB-4, and which of the standard causes have been excluded.
