The numbers, and where the ambiguity comes from
The Office of Dietary Supplements sets out the problem in a single sentence in its health professional fact sheet: serum or plasma B12 levels are typically used to assess status, with values lower than 200 or 250 pg/mL generally considered subnormal, and the most sensitive marker is a B12-associated metabolite called methylmalonic acid, which can confirm a diagnosis of deficiency when serum B12 sits between 150 and 399 pg/mL.
Read that carefully and the gray zone is explicit in the official source. A serum B12 of 350 pg/mL is above every subnormal cutoff, will be printed as normal, and is inside the range in which a second test is recognized as capable of confirming deficiency. That is not a fringe position; it is the federal nutrition authority's own description of the test's limits.

Why the symptoms are so easy to dismiss
The ODS list of deficiency symptoms reads like a list of reasons people do not go to the doctor: fatigue, neurological changes, megaloblastic anemia, inflammation of the tongue, palpitations, and low counts of white and red blood cells. And then the sentence that explains the diagnostic delay: the symptoms of vitamin B12 deficiency can take several years to appear.
Two consequences follow. A single normal result years ago says little about now, particularly if you have started metformin or an acid-suppressing drug since. And the anemia that gave the deficiency its classical name is not reliably the first thing to show up, so waiting for a low hemoglobin before considering B12 is the wrong order. Who is actually at risk
ODS names the groups more likely to be deficient, and they are worth checking yourself against because most are not obvious.
- Older adults. Absorption from food falls with age, largely through reduced stomach acid, which is needed to release B12 from protein.
- People with pernicious anemia. An autoimmune loss of intrinsic factor, the protein required to absorb B12 at all. Dietary intake cannot compensate.
- People with gastrointestinal disorders or previous gastrointestinal surgery. Celiac disease, Crohn's disease, gastric bypass and sleeve gastrectomy all reduce absorption.
- People on vegetarian and vegan diets, and infants of vegan mothers. B12 is essentially absent from unfortified plant foods.
Separately, ODS notes that some medicines affect B12 levels, naming gastric acid inhibitors and metformin. Neither is a reason to stop a drug that is working, and both are reasons to check the level rather than assume. If you have been on metformin or a proton pump inhibitor for years and have never had B12 measured, that is a straightforward gap to close.

What supplementation does and does not do
B12 has a reputation as an energy supplement, and the evidence does not support that use. ODS states that B12 supplementation does not appear to improve athletic performance or endurance in people who have sufficient B12 status. The nuance is in the last clause: correcting a real deficiency can transform how someone feels, while topping up an adequate level does nothing.
The cognitive story is similar and more disappointing. Observational studies have found correlations between low B12 and poor cognitive function, but ODS reports that randomized controlled trials have shown no improvement in cognitive function among older adults with or without dementia, mild cognitive impairment or Alzheimer's disease when supplemented with B12 alone or with folic acid, vitamin B6, or both. The recommended intakes are modest. ODS puts the adult values in the range of 2.4 to 2.8 micrograms per day, so this is not a nutrient where large doses are needed to meet requirements. High-dose oral treatment is used in deficiency because only a small fraction is absorbed passively when the normal absorption route has failed, which is a treatment strategy rather than a target intake.
What to do with a borderline result
Ask for methylmalonic acid if your B12 is between roughly 150 and 400 pg/mL and you have symptoms that fit. It is the step that converts an ambiguous number into an answer.
Say what you take. Metformin, proton pump inhibitors, H2 blockers, and any B-complex or energy supplement, which can raise the serum level while the underlying absorption problem persists.
Ask why, not just how much. Deficiency from a vegan diet is corrected differently from deficiency caused by loss of intrinsic factor, which usually needs lifelong replacement.
Get red cell size from the same panel. Enlarged red cells alongside a gray-zone B12 strengthen the case considerably.

Frequently asked questions
Is a B12 level of 300 normal?
It is above the subnormal cutoffs, so it will be reported as normal, and it also sits inside the 150 to 399 pg/mL range in which the NIH says methylmalonic acid can confirm a deficiency. If you have no symptoms, 300 is reassuring. If you have symptoms that fit and a risk factor such as metformin use or a vegan diet, it is a reason to ask for MMA rather than to stop looking.
Can B12 deficiency cause nerve symptoms without anemia?
Yes. Neurological changes are listed among the deficiency symptoms in their own right, and the ODS notes symptoms can take several years to appear, so relying on a low hemoglobin as the trigger for testing misses cases.
Do I need injections or will tablets work?
It depends on why you are deficient. Where absorption is intact, oral replacement is usually effective. Where intrinsic factor is missing, as in pernicious anemia, or where the relevant part of the bowel has been removed, injections or high-dose oral regimens supervised by a clinician are the usual approach. This is a decision to make with the person who knows the cause.
Should I take B12 for energy?
Not unless you are deficient. The NIH states that supplementation does not appear to improve performance or endurance in people whose B12 status is already sufficient.
Can too much B12 be harmful?
B12 has no established upper intake level, and excess is generally excreted. A high measured level is more often a clue than a problem, because it can accompany supplement use, and in some cases liver or blood disorders, which is why an unexplained high result is worth mentioning rather than ignoring.
When to talk with a clinician
Raise B12 specifically if you have numbness, tingling, unsteadiness, memory change or a sore tongue, if you are on long-term metformin or acid suppression, if you follow a vegan diet without supplementing, or if you have had gastrointestinal surgery.
