What to know before you sign up

LillyDirect is not a weight-loss program. It is Eli Lilly's own pharmacy channel, and the self-pay prices on its Zepbound page are the reference every brand-tirzepatide program in this table is measured against: $299 a month for the 2.5 mg starting dose in a vial or KwikPen, $399 for 5 mg, and $449 for 7.5 mg and above when you refill within 45 days, with a month defined as 28 days. Miss that refill window and the regular price applies, $499 for 7.5 mg and $699 for 10 mg and up. Foundayo, Lilly's daily pill, starts at $149 a month for the 0.8 mg dose and reaches $299 at the higher doses (at 14.5 mg and 17.2 mg only when you refill within 45 days; the 2.5 mg and 5.5 mg prices are temporarily reduced through December 31, 2026).

What LillyDirect does not provide is a prescriber. Step one on its page is "Ask your doctor to send your prescription to LillyDirect Pharmacy", and the find-a-doctor link goes to independent telehealth providers whose fees Lilly does not print and whose availability "varies by state and condition". So the fair comparison is LillyDirect's drug price plus whatever your own clinician or a telehealth visit costs, against a program's membership plus its drug price.

The single-dose vial is sold only through LillyDirect and only self-pay. The single-dose pen takes commercial insurance with a savings card that can bring the copay to $25, and government beneficiaries are excluded from that card. Every price here is a savings-program price that Lilly says it can amend or end at any time, so read the terms on the day you order.

How much does Zepbound cost through LillyDirect?

On September 23, 2026 the self-pay price was $299 a month for the 2.5 mg vial or KwikPen, $399 for 5 mg and $449 for 7.5 mg through 15 mg when refilled within 45 days ($499 to $699 otherwise). A month is 28 days. The single-dose pen is an insurance product with a $25 copay card for eligible commercial plans.

Has the FDA or a regulator taken action against Eli Lilly?

Yes, on record as of September 23, 2026: three FDA warning letters dated September 9, 2025 about Zepbound promotional videos found false or misleading, and 2026 state attorney general suits in Delaware and Florida over insulin and diabetes-drug pricing. Two of them concern videos that promoted Zepbound through LillyDirect without risk information; none concerns LillyDirect's dispensing, and the FTC case library returned nothing.
